Dental: CCCBA’s Dental Plan, underwritten by Lincoln Financial Group, offers two different calendar year maximum options that are available to firms with two or more enrolled attorneys and/or employees. Both plans allow you to choose any dentist; however, you can maximize your benefits by utilizing PPO providers. Firms with at least five enrolled employees do not have a waiting period for Major work. In addition, there is an Indemnity (go to any dentist) plan with a higher calendar-year maximum for those firms with at least fifteen attorneys and/or employees enrolled.

Vision: CCCBA’s Vision Plan, underwritten by MES/Blue Shield Life Insurance Company, provides for eye exams and eyewear (including contact lenses). Members may use any optometrist or ophthalmologist but will receive higher benefits when they use participating providers or vendors.

Life and Accidental Death & Dismemberment: CCCBA members may apply for up to $2,000,000 with super-preferred rates on amounts of more than $500,000 (Available at age 30 and up). Coverage is guaranteed renewable to age 75 and convertible to a permanent policy anytime before the policy terminates at age 75. This plan includes an accelerated life benefit payout for terminal illness. CCCBA members’ spouses are able to apply for coverage if the member does not apply.
